Study Overview : e-SAATHI

A Digital Companion for the Maternal Health Journey

e-Saathi is a digital, chat-based maternal health intervention designed to support women across the entire pregnancy and postnatal journey. It integrates digital health, self-care, and health financing pathways to improve access, awareness, and utilization of maternal health services.

he platform enables continuous engagement with women by delivering timely, personalized, and stage-specific information, helping them navigate complex healthcare systems and make informed decisions. At its core, e-Saathi aims to bridge the gap between health systems and women’s lived realities, particularly in resource-constrained settings.

The Problem

Barriers That Limit Access to Maternal Care

img

spite significant policy efforts, maternal health outcomes in India remain uneven, especially in underserved regions. A large proportion of women continue to face systemic and demand-side barriers that limit access to quality care.

Key challenges include:

  • High out-of-pocket expenditure for maternal health services
  • Limited awareness of government schemes and entitlements
  • Low autonomy in healthcare decision-making
  • Gaps in implementation of pro-poor policies
  • Poor continuity of care across ANC and PNC stages

These barriers significantly impact timely care-seeking, adherence, and overall maternal health outcomes, highlighting the need for integrated, scalable solutions.

Objective

Improving Access, Awareness, and Maternal Outcomes

The e-Saathi initiative is designed to strengthen maternal health outcomes through a multi-dimensional approach.

The key objectives are to:

  • Improve women’s access to timely and relevant maternal health information
  • Increase uptake of antenatal (ANC) and postnatal (PNC) services
  • Strengthen self-care practices and health awareness
  • Enhance women’s autonomy and decision-making capacity

Enable better navigation across public and private healthcare systems

Study Design and Methodology

Study Design

An Integrated Approach to Digital Maternal Health

The e-Saathi model is built on the intersection of three critical domains:

01

Health Financing: Leveraging existing pro-poor maternal health policies

02

Digital Health: Chat-based engagement and mobile health ecosystems

03

Self-Care: Empowering women with knowledge and agency

The intervention is implemented through a chat-based digital platform (AskNivi) that delivers:

  • Stage-specific health information
  • Reminders for care and preventive actions
  • Referrals to nearby healthcare providers
  • Feedback mechanisms to capture patient experience

This approach enables continuous, real-time engagement, ensuring that women receive support throughout their maternal journey.

Intervention:

  • Gestation-based health messaging tailored to each stage
  • Reminders for ANC, PNC, and preventive care practices
  • Information on financial incentives and maternal health schemes
  • Referrals to nearby public and private healthcare providers
  • Feedback collection to improve care delivery

Outcome Measures:

  • Primary measures:

    • Uptake of antenatal and postnatal care services
    • Improvement in maternal health awareness
  • Secondary measures:

    • Adoption of self-care practices
    • Patient engagement and interaction levels
  • Access to healthcare providers across systems

Key Findings

Digital Engagement Improves Care-Seeking Behavior

Evidence from the study highlights the transformative potential of digital health interventions in maternal care.

Key insights include:

  • Improved maternal health awareness and knowledge levels
  • Increased engagement and interaction with healthcare systems
  • Better adherence to recommended care practices
  • Enhanced patient satisfaction and perceived quality of care
  • Persistent gaps in awareness of government schemes, indicating need for integration

These findings reinforce the importance of combining digital tools with behavioral and system-level interventions.

Strengthening Maternal Care Beyond the Clinic

e-Saathi is designed to create meaningful, real-world impact across the maternal health ecosystem.

Key impact areas:

  • Improved care-seeking behavior and service utilization
  • Increased maternal awareness and autonomy
  • Better navigation across healthcare systems
  • Strengthened linkages between patients and providers
  • Reduction in delays in accessing care

The intervention shifts care delivery from episodic interactions to continuous engagement.

Highlight

Timely, personalized digital support can transform maternal health outcomes at scale

Global Significance

A Scalable Model for Maternal Health Systems Worldwide

The e-Saathi model offers a scalable and adaptable solution for addressing maternal health challenges globally. Its broader relevance includes:

01

Applicability in low- and middle-income countries (LMICs)

02

Alignment with global priorities on women’s empowerment and digital health

03

Demonstration of integrating digital, financial, and self-care interventions

04

Potential to inform national programs and global health strategies

It presents a pathway toward more equitable, accessible, and patient-centered healthcare systems.
